- In home visit within 24-96 hours of discharge of scheduled surgery (Up to 2 hours of Nursing care per visit)
- Wound Care and Dressing Changes: Monitoring surgical sites for signs of infection, changing dressings, and providing wound care.
- Medication Management: Assisting with medication schedules, ensuring you understand prescribed medications, and monitoring for side effects.
- Pain Management: Providing guidance on managing pain through non-pharmacological methods and ensuring adherence to prescribed pain management plans.
- Mobility Assistance: Encouraging and assisting with mobility exercises to prevent complications such as deep vein thrombosis (DVT) and to promote circulation.
- Nutritional Support: Offering advice on nutrition that supports healing, including hydration, vitamins, and minerals crucial for recovery.
- Patient Education: Education on care procedures, what to expect during recovery, signs of potential complications, and when to seek medical attention.

- Coordinating and preparing for hospital stay (preparing hospital bag, setting up transportation, discussing doctor's pre-operational and post birth orders, re-enforcing doctor's orders and paperwork)
- Newborn Care preparation (bottle feeding and breastfeeding, safe sleep, tips on organizing your day to help reduce stress levels)
- Neonatal comfort measures preparatory training
- Nurse led Baby showers
- One-Two hour Relief visits (Number of visits is dependent on your plan)-These visits are to help you obtain some personal time in your day while we watch your baby and you do what you desire to do.
Special Baby (Pre & post birth Concierge Nursing Support
- Specifically designed for high-Risk babies, NICU babies and moms with a high-risk pregnancy determination
- High Risk/High Acuity, as defined by your doctor, Pre-birth planning support concierge Nursing visits to help you get set up, organized and prepared for baby's arrival (Assessment of supplies needed as well as support with organizing supplies needed for baby's arrival)
- Coordinating and preparing for hospital stay (preparing hospital bag, setting up transportation, discussing doctor's pre-operational orders, re-enforcing doctor's orders and paperwork)
- Newborn Care preparation (bottle feeding and breastfeeding, safe sleep, tips on organizing your day to help reduce stress levels)
- Post birth transition from hospital to hospital to home by helping ensure discharge orders are understood and ensure special equipment, appointments and medications are all set up
- Neonatal comfort measures preparatory training
- Nurse led baby showers
- One-Two-hour relief visits (Number of visits are dependent on your plan)-These visits are to help you obtain some personal time in your day while we watch your baby, and you do what you desire to do.

Seniors Wellness Visits
- This service is provided on a weekly biweekly or monthly basis with the aim of ensuring the long-term well-being of your loved ones, preventing hospitalization. I will monitor your family member(s) or loved ones regularly and provide updates as necessary. As a participant, you will benefit from the following services:
- An in-home visit/wellness check
- Monitoring of blood pressure, temperature, heart rate, and oxygen levels
- Assessments of heart, lung, circulation, and mental status
- Evaluation of the home environment for safety
- Examination of medical reports, upcoming appointments, and organization thereof
- Education on medications, assistance with medication administration, scheduling if required, and review
- Review of medical tests
- Education on health-related matters
- Assistance in scheduling appointments when necessary
- Liaison with the medical team as needed
- Regular updates to the family as required
- Providing companionship
Seniors Life Wellness and placement coordination
- Home Wellness Assessment
- Assistance in securing home care aids or retirement homes depending on needs and desires.
- Professional assessment of which setting (home or nursing home) will be safe and appropriate.
- Assistance in locating the right retirement place or home care agency
- Education on health-related matters
- Liaison with the medical team for placement finding
- Regular updates to the family as required
Newborn/Pediatric Wellness Visits
- This service is provided on a weekly biweekly or monthly basis with the aim of ensuring the long-term well-being of your loved ones, preventing hospitalization. I will monitor your newborn or child regularly and provide updates as necessary. As a participant, you will benefit from the following services:
